List of documents to acquire the secondary specialization:

• application for secondary specialization;
• written consent to the processing of personal data;
• copy and original of the Specialist’s/Master’s Diploma;
• copy and original of the Medical Specialist Certificate;
• 2 passport-size photos;
• copy and original of the taxpayer card (identification code);
• copy and original of the Ukrainian passport (ID card);
• marriage certificate if the diploma is issued to another surname.
